New Patient Forms

As in any doctor’s office, there are forms to fill out.  We have placed our New Patient Forms on our website for your convenience.  These forms require Adobe Acrobat Reader software, which most likely is already installed on your computer.  If you do not have the software you may download Adobe Acrobat Reader here:  

Please print and complete them at your leisure prior to your appointment with the surgeon.  The forms will require approximately 45 minutes to complete.

In addition to new patient forms we have also provided the preparatory surgery guide, and a document discussing how expenses toward weight loss surgery can be TAX Deductible.
